Service No 01 : Registration of Mosques and Appointment of Mosque Trustees

1.1 Eligibility :

  • The mosque should have been established before a minimum period of six months
  • Mosque Trustees should take necessary action to register all the mosques under the section No 10(1), 10 (2) of Wakf Act

I. Documents for Mosque Registration

  • Main application for registration (‘A’ Form)
  • Certified copy of the title deed
  • Title Deed/Pedigree/Extract
  • If a government land, an approved letter from the Divisional Secretary
  • Certified Copy of the Plan of the Land
  • Prior approval obtained from the government authorities to build the Mosque/Copy of the Approved Building map
  • If the land belongs to the trust, a recommendation letter of the Trustee board of mosque/Zaviya/Thakkiya should be submitted.
  • If there is any special statement of the person who wakf the mosque, related document should be submitted.
  • Constitution of the Mosque (Click here for a Sample Constitution)
  • List of Fixed assets which belongs to the Mosque
  • Annual Accounts statement of the mosque since established year (Click here to obtain the Accounts Submission form)
  • No Objection letter from the closest Jumma Mosque to register the Mosque.
  • A Letter of the Grama Niladari attested by the Divisional Secretary regarding the mosque registration.
  • Inventory List
  • Minimum 05 photographs of the Mosque
  • Copies of the Electricity and Water bills (If available only)

II. Documents of Trustees Appointment

  • Name List of Jamath members (Gents only) above 18 years old (Name, Address and NIC No. should be included)
  • Meeting Minutes of the meetings held for appointing new Trustees.
  • Attendance List - Name list along with signatures of the persons in jamath who participated in the above meeting.
  • Declaration Form
  • Petition Form
  • Affidavit
  • Certificate on residence and character issued by the Grama Niladhari
  • Police Clearance Report about the Trustees
  • Affidavit certified by an Attorney-at-Law that the particular Mosque and its Trustees are not involved with any organization banned by the government of Sri Lanka.
  • A cheque/money order worth of Rs.20.00, drawn under the name of Muslim charities Fund
  • A photo copy of all the above original documents should be submitted along with the originals.

  • Details of the students who have passed out Al Aalim Primary/Final/G.C.E. O/L/ G.C.E. A/L in this |Arabic College.
  • Accounts Report of the College since the commencement of the College.
  • Name and Adresses of thee Sponsers/Donors of college and Members of College Committee
  • List of documents and furniture which are being used at present
  • Activities which are recently took place.
  • List of the participants who took part in meeting held for selecting the administrative officers of the college. (with their signatures)
  • Disciplinary manual of the college for the Teachers and the Students
  • Snaps of the Front view, Classrooms, Toilets, Bathrooms and Hostels etc should be submitted with the rubber stamp placed behind the snaps
